If your Health Care Professional is NOT participating in the Homeopathic Proving:

1. EMAIL: referral@integrahealth.net to obtain physician referral.

If your Health Care Professional is participating in the Homeopathic Proving:

1. ARRANGE Appointment to begin homeopathic care.

2. REGISTER and COMPLETE the Consent Agreement for Homeopathic Care available at the office of your sponsoring Health Care Professional.

3. FAX Registration and Donation Fee (Visa, MC) to IntegraHEALTH.net (720) 306-3720

4. COMPLETE and SUBMIT Health Questionnaire online at www.IntegraHEALTH.net

If your Health Care Professional is interested in knowing more about the
Homeopathic Proving:

1. VISIT the Home Page

2. CLICK the “Science Library Site” button

3. PRINT & DELIVER the Technical Review to your Physician, and

4. ASK him or her to visit : www.integraHEALTH.info / hcp
